COVID CONSENT AND WAIVER:

THIS CONSENT CONTAINS A WAIVER OF LIABILITY AND RELEASE OF BOYS & GIRLS CLUBS BIG BROTHERS BIG SISTERS OF EDMONTON & AREA SOCIETY AND ITS AFFILIATES. PLEASE READ THE DOCUMENT AS IT CONTAINS IMPORTANT INFORMATION REGARDING YOUR LEGAL RIGHTS, LIMITATIONS THEREOF AND YOUR ASSUMPTION OF ALL RISKS ASSOCIATED WITH PARTICIPATION IN THE IN SCHOOL MENTORING PROGRAM

The information being collected by the Boys & Girls Clubs Big Brothers Big Sisters of Edmonton & Area Society is being collected for the express purpose of operating the In School Mentoring Program and will be used and may be disclosed to staff and volunteers as required for such purposes.

I am fully aware and agree to the following:

ASSUMPTION OF RISK, WAIVER AND RELEASE AGREEMENT

While the Boys & Girls Clubs Big Brothers Big Sisters of Edmonton & Area Society takes reasonable steps as recommended by the Alberta Government to avoid the transmission of Covid-19, I understand and accept that my child's participation in the In School Mentoring program (the "Program") exposes my child to certain risks including the risk of contracting Covid-19 which may result in serious illness or death and I assume and accept full responsibility for suck risks.

Waiver and Release: In consideration of Boys & Girls Clubs Big Brothers Big Sisters of Edmonton & Area Society offering services to my child through the Program, I specifically release and forever discharge Boys & Girls Clubs Big Brothers Big Sisters of Edmonton & Area Society and its parent, subsidiary and affiliated corporations, directors, officers, agents, and employees (collectively the "Releasee") from any and all liability or claims for injury, illness, death, or loss of or damage to property which my child may suffer as a result of participation in the Program. This discharge specifically includes, but is not limited to, liability or claims for injury, illness, death or damage caused by the negligence of the Releasee. It is my intent by this Agreement to release the Releasee and hold it harmless from all liability for any such property loss or damage, personal injury, illness or loss of life. whether caused by the negligence of the Releasee or whether based upon breach of contract, breach of warranty, or any other legal theory. In signing this document I fully recognize that if injury, illness, death or damage to property occurs to my child as a result of my child's participation in the Program, I will have no right to make a claim or file a lawsuit against the Releasee, even if they or any of them negligently cause my child's injury, illness, death or damage to my child's property.
